1) You must be an active patient in Dr. Morris' Miami Lakes office (sorry, no e-mail accepted for Dr. Bierman's or Dr. Cody's patients). If you are not a current patient, the e-mail will be deleted without being read (please no attachments).

2) All e-mail must originate and terminate from with in the State of Florida (no out of state e-mails will be accepted)

3) The office must have a signed consent for any e-mail communications (click here for copy of consent).
